1. Who is eligible for the Program?

The TIDE Ticket to Ride Taxi Voucher Program is available for residents of Hanover Township, Cook County, 16 years or older, who have a physical, cognitive (mental) or developmental condition that interferes in at least one "major life activity". For the purpose of TIDE, the "major life activity" is the ability to be self-transporting (make arrangements for and travel by one's self) over distances. The TIDE Ticket to Ride Voucher Program is available for cab rides to or from work or job training only.



2. Are there geographic boundaries for the Program?

The TIDE Ticket to Ride program will provide transportation to work sites up to 7 miles outside the boundaries of Hanover Township. Hanover Township boundaries extend North to the 90, South to Devon, and East to Barrington Road and West to Hiawatha Drive. The Cook County portions of Elgin, Bartlett, Hanover Park and Streamwood are part of the Township. In addition, a small portion of Hoffman Estates and Schaumburg are within Hanover Township Boundaries.




3. How do I apply for the TIDE Ticket to Ride Program?

    You can either apply:

  • Through the mental health board offices. Call (630) 837-0301 to schedule an appointment. Office hours are 8:30 am-4:30 PM. Appointments are recommended. Call (630) 837-0301 to schedule the appointment.
  • You may also fax an application back at (630) 837-0068.


4. Who completes the application form?

The person with the disability who needs to get to work or job training fills out the form, if s/he is an adult (18 years or older) and does not have a legal guardian. If the person with the disability is between the ages of 16 and 18 years, or if the person has had a legal guardian, then the parent or legal guardian will complete the application form.




5.  What information needs verification?
  • Month, day and year of birth
  • Township residency
  • Disability information - Verification from 2 sources is required. Acceptable sources include, but are not limited to, physicians, nurses, professional staff from social service agencies, school personnel, physical or occupational therapists, independent living specialist, rehabilitation specialist, ophthalmologist or mobility instructor.
  • Work or job location and schedule (hours that you work) (name and telephone number of work or job site supervisor is required.)


6. What will be the cost?

The cost of each voucher will be determined on an individual rider basis based upon the mileage to or from the work site. The rate is $3.80 for the first mile and $2.40/mile for each following mile. Participants will pay ½ the cost of the voucher. We will determine the amount once we have "processed" your application. For example: it is 3 miles to work. The cost of the trip will be $8.60. This amount will appear on your voucher. You will pay $4.30 per voucher for your ride to work or job training. Payment will be made to the Hanover Township Mental Health Board.




7. How much money should I bring for the ride?

No money is required. The voucher covers the total cost of the ride, including tip. If the driver asks for additional money, please let Danise at the Hanover Township Mental Health Board know immediately! Her number is (630) 837-0301.




8. Why would an applicant be ineligible for the Ticket to Ride Program?

  • Living outside of Hanover Township
  • Being under the age of 16 Transportation need is not related to work or job training
  • Not having a physical, developmental or cognitive/mental disability
  • Being unable to secure a seat belt without driver assistance
  • Providing false information on the application



9. Are there any other transportation services available for persons with disabilities through Hanover Township?

Transportation services for persons with disabilities are available through the Hanover Township Senior Services Department. These services are available Mon-Fri. These services are available for persons needing to get to doctor's appointments, shopping and other activities. People applying for the TIDE Ticket to Ride Program will have their information forwarded on to the Transportation Supervisor. 10. How will I know if I’m accepted in the program?

You will be contacted by phone or in writing. If you are approved, you can come to the Township Office at 250 S. Route 59 in Bartlett to obtain your information packet and vouchers. If you are denied, you will receive, in writing, the reasons for this decision. You have the right to appeal this decision in writing. You begin the process by contacting Danise Habun, Director of Community Mental Health Services for the Hanover Township Mental Health Board. The steps to the appeal process will be provided at that time.
